Medication self management support for patients with limited health literacy

bleodsuikermeter
The project examines how patients with a chronic disease and limited health literacy can be supported in their medication self-management.

Objective

The aim of the project is to improve medication self-management of people with a chronic disease and limited health literacy.

Results

The result is to gain insight into the medication self-management support needs of people with limited health skills and a chronic disease and to gain insight into which support meets those needs.

Duration

01 January 2018 - 01 January 2023

Approach

The project starts with a review of existing interventions for people with limited health literacy. The needs of people with limited health literacy are then identified and an intervention that meets those needs is tested for usability for the patient. It is also tested to what extent healthcare providers offer the intervention to people with limited health literacy.
